All information I have provided to Gamers Helping Gamers, Inc. is complete and accurate to the best of my knowledge. I understand that falsification of information will result in termination of any scholarship(s) granted. I hereby give Gamers Helping Gamers, Inc. permission to share this information for the purpose of promoting its organization and scholarship program. Specific information related to an applicant's financial status is not covered by this grant of permission and will not be shared by Gamers Helping Gamers, Inc.

I certify that I will be enrolled as a student for the upcoming academic year and will provide proof of enrollment or other documentation requested by Gamers Helping Gamers, Inc. If I am selected for a scholarship, I will use the award toward expenses related to my attendance.

I certify that I am not related to any officer, director or substantial contributor to Gamers Helping Gamers, Inc.

All application materials become property of Gamers Helping Gamers, Inc.
